Here is the answer clip to this challenging case of duodenal perforation…the lateral radiograph was already pointing to this direction, but it did not alert the clinician initially.
Pocket of gas, debris of foreign material and echogenic fluid can be difficult to distinguish from the inflamed mesentery. In my opinion, trapped gas bubbles mixed with leaked fluid is harder than free gas to detect on ultrasound…

Necrotizing Right limb pancreatitis (most likely) vs. carcinamatosis vs. open - Febrile, BP/HR/RR, CBC w/blood smear and UA, chest and abdominal radiograph results, acid/base/lactate are all important results to know, next perform abdominocentesis for culture and cytospin prep cytology STAT!!!

The changes are severe. Small bowel affection, probably duodenal. I believe to see intestinal wall thickening with mural hypoechogenicity and loss of layering. Pancreatic edema?. Hiperechoic peritoneal effusion, severe hyperechoic oment/steatitis and free gas?. Enphysematous pancreatitis?. Suspected digestive perforation and finally, a possible free peritoneal foreign body?
Would it be precipitated to recommend exploratory laparotomy?.
Sorry by my comments.

The large structure in center is consistent with the stomach, probably the area of the pylorus with the pyloric valve visible. There is a hyperechoic structure with complete anechoic shadow suggesting a foreign body. Additional questions would be about: durations of symptoms; any history (current and past behavior) of eating of chewing something that isn’t food; any vomit and what did it look like. Next steps: x-ray might show if radio dense; lab work and exploratory.

Páncreas is the main organ affected? I can see ecogenic effusion, gas, mineralization focus?? meso reactive, duodeno inflammated.
I would like to know what did he eat lattely? If he if painful ? When did the clinical signs began? Because it looks like it has a few days of evolution..
My next step will be having a sample of the free fluid liquid
Is it pancratitis necrotizante, acute pancreatitis?
