Why is the dog straining?
1 · The case
Lets start 2018 with the exam performed on Frankie, a 4-year-old male Pit Bull presented for straining to urinate and defecate. He was given 2 weeks of Baytril and his clinical signs resolved. He was scheduled for an internal medicine consultation and an ultrasound was performed. What is the likely diagnosis? What would you recommend?

Paraprostatic abcess/cyst?
could you please show the same images but with color doppler? I would sedate the patient and perform an aspirate. I would also send the sample for a culture and citology, continue on Baytril for at least 3 more weeks and do a follow up ultrasound. If the patient is not neutered I will strongly recommend it too. In case that in the follow up visit the ultrasonographic abnormalities remains as before, I´ll suggest an exploratory laparotomy in order to perform an omentalizacion of the prostate, biopsy-culture.

I think Prostatic abcess or hematoma ?? the prostate echotexture seems heterogeneous and with rounded anechoic structures , may be prostatic hyperplasia also?.

BPH and prostatitis with several small parenchymal cysts and a large paraprostatic cyst. Multiple FNA's and cytology/ culture should be performed. The cyst should be surgically excised. Patient should be neutered.

Bladder appears well distended. Bladder walls appear thin. Prostate is enlarged with several cystic areas within the prostate. Large cyst appears to arise from the right cranial aspect of the prostate. The prostatic cyst has a fluid level with echoes in the dependant portion of the cyst. I cannot comment on the treatment.

Severe BPH vs. Prostatitis with a large paraprostatic cyst with probable gravity-dependent hyperechoic blood in the cyst. There are multiple small cysts throughout the prostate that may represent abscesses. I would use ultrasound-guided cavity centesis to drain the cyst for therapeutic benefit as well as culture & sensitivity, collect fine-needle biopsies mostly for culture as the odds of this being neoplasia are very low but cytology will rule-out neoplasia. Then while on antibiotics, castrate with recheck ultrasound in 2 weeks and again at 1 month to monitor whether the cyst needs to be drained again.

I've read the replies and have a query. Would it not be risky taking an FNA of something which could be an abscess, seeding infection outside the prostate? Thanks for any thoughts.

I would like to find the kidney, because I know the prostate is enlarged and not homogeneous, but there's a structure that resembles an altered kidney..did he had normal urine? without blood? feveR?

Paraprostatic cyst.Enlarged vessels within both lobes of prostate? Is there change in the prostatic part of urethra? looks bit abnormal? Thank you for this case!

Large Prostate with a large cyst on the anterior right side. I assume this is an unneutered male. I would want a UA with a culture/sensitivity, CBC and Profile. A FNA of the cyst with culture would be great but rupture of the cyst would have to considered. If the UA shows the infection, then I would pass on the FNA. Treatment would include neutering, antibiotics for 4-6 weeks minimum with US and clinical resolution before stopping Ab Tx. Young dog for prostatic disease.
