Creo que hay gas en el espesor de la pared gástrica
Steve Ting, DVM
In the first part of the clip there is gastric intramural gas shadowing present. There is a large amount of gastric content. Most of which does not shadow, though there are foci of shadowing material present. Towards the end of the video clip there is a hyperechoic linear structure that casts a hard shadow near the end of the clip, This structure looks like it is very near to or has already perforated the stomach near the very and of the clip.
The intramural gas may indicate ulceration or damage from a foreign body. The linear structure is very concerning for a foreign body, that is very close to perforating the gastric wall.
CT could be considered for further information. Assuming the small bowel is normal can consider endoscopy though the amount of material in the stomach may hamper visualization. If endoscopy is unsuccessful, then exploratory surgery is likely indicated. Exploratory may be the safest option as it would be the best way to evaluate the gastric wall for perforation.

Penetrating foreing body, mineralization of the stomach wall secondary to the inflamatory process

Layers of gastric wall more pronounced, and the intermittent shadowing seen within the gastric wall would suggest gas pocketing. the contents of the stomach appear relatively homogenous and not moving. One possibility would be gas producing infection of the stomach wall with bleeding and the contents are possibly a large blood clot.

Stomach is filled with echogenic fluid, some gas and a long, thin, smooth surface which seems to be folded. There is intramural gas in the gastric wall, which I believe can be of no clinical significance, but since there is a foreign body and vomiting it might suggest some degree of disruption of the gastric wall. Suggest endoscopy or surgery.
Thank you for the case and regards

Hi. i think that could be a gastritis with ulcers. Endoscopy should be the next thing to do. Thank you

The wall is a little thickened. There appears to be suspect wall ulcerations that are not full thickness. Stomach content is moderate with material that could be consistent with food. What kind of material is he vomiting and when the last meal? Why is there still material present with vomiting. Xrays might be helpful and recheck US in a few hours.

There is gas in submucosal layer? Emphysematous gastritis exists? secondary to gastric ulceration?.
The gastric content seems normal.

Gastric pneumatosis and foreign body (skewer pick?) in the gastric lumen?
Thank you
sheri
I suspect there is gas in the submucosal layer of the wall. There is an indentation of the mucosal surface with gas. the lumenal contents have a hyperechoic linear structure that casts acoustic shadow. There is also a curvilinear acoustic shadowing structure within the lumen. Suspect gastric foreign body(ies) , penetrating?, ulcerating . Radiographs and endoscopy would be complimentary.
