The features seem uretherocele, right uretheral dilation and pyelectasia. I think they may cause recurrent ITU and sphincter incompetence by pressure?.

Right kidney : hydronephrosis, dilation of the right ureter and ureterocele of the right ureter. surprising, seeing the age of the dog, that he has not been diagnosed at a younger age.

Extended renal pelvis, hydronephrosis, renal damage and ureteral distension due to blocked right ureteral bladder opening. There's a structure in bladder trigon or ectopic right ureter so cystoscopy is needed here. This bladder cannot evaquate appropriately so infections frequently occure. No signs of severe cystitis.
